Admissions Appeal Form

If you have not met the automatic requirements for admission and have received a letter requesting additional information, you may complete and submit this appeal form to be reviewed by the Admissions Review Committee. The committee conducts weekly review sessions to ensure timely follow-up communication to students awaiting an admissions decision. Students admitted through the Admissions Review Committee may be subject to a conditional enrollment plan or other requirements as determined by the committee.
